It's a tiny invisible microphone/speaker that clips to a rear molar. It picks up a person's voice and conducts sound to their ear through their tooth and skull. They say that it's especially good for extremely noisy environments.

It's called 'Molar Mic' manufactured by a company called Sonitus Technologies, in San Mateo not far from my home!
