It's a tiny invisible microphone/speaker that clips to a rear molar. It picks up a person's voice and conducts sound to their ear through their tooth and skull. They say that it's especially good for extremely noisy environments.

It's a project of the Department of Defense's very cool Defense Innovation Unit, kind of a venture capital firm for outside-the-box military technology.
