Yep, in some cases it may be safer to just improvise like that, medical issue or not. If an establishment has interior restrooms located in raucous corners where cliques of guys hang out or the external ones are on a seedier, dark or barely lit side, then the place might as well not have any at all.
Lack of facilities in India, along with the country's double standard, is especially a barrier for girls.

We girls got used to it, to holding back for hours. It’s just how we grew up. But for girls in rural areas and urban slums, lack of access to clean toilets and sanitation is a big cause of dropping out of school, or not going to school at all. It’s the difference between education and independence and being locked into poverty and subjugation.
Men seem to feel no such constraints. In India, men pee everywhere, even in urban areas. Middle class, educated men — if they have to go, they’ll just stop their car and pee against a wall. Or even defecate on the sidewalk.
I do recall a male employee who almost got in trouble when he had to defecate in the open due to the lone toilet left in the office building even going out of order. He'd lost half of his entrails years before from a gunshot wound, so he couldn't hold it in long enough to reach the alternative down the block that everybody else was going to. Those particular company grounds were located in the middle of a residential neighborhood, of all things. He tried to hide behind a fork-lift, but the locals still saw him and called the police. But once the manager explained to them the circumstances, they let the incident go.

strong self assured women being a threat or un-sexy etc etc...
equally women in varying states of general life issues being defined as un-clean and/or not "sexy" and/or thus not desirable(as if that's the worst possible position to be in in life as a women as men hold it over them etc etc ).
Difficult to imagine Trump ever asking "What needs to be done?" as opposed to "Here's what I think should be done..." at a gathering of the heads. If he was a woman doing the latter, he'd get whisperingly accused of "female solipsism".
