Some kids of today want to transition because they believe that will solve their depression or difficulty with social interactions or misfit status, etc. Whereas in the old days of therapy, the boy might have been told to get over his Oedipus complex or the girl to resolve her penis envy issues.
An adult like this guy probably tried every snake oil treatment and "healing ritual" for his psychological troubles around, over the years. Before finally self-diagnosing himself as needing to be a woman, albeit he also needed an expert to concur. ("All that's left, it's got to be the magic solution!")
